Permissions & Acknowledgements *

By submitting this form, I am agreeing to the following.

1) I understand that my student may participate in physical activities, during games or activity time. As with any physical activity there is a risk of injury. I fully accept this risk and hold harmless from any legal liability, Belgrade Alliance Church and any persons involved in Awana Ministry at Belgrade Alliance Church.

2) In the event of an emergency that requires professional medical treatment for my child, I understand that every effort will be made to contact me or my emergency contact. However, if I/we cannot be reached, I give permission to Awana Ministry volunteers & staff to secure the services of a licensed physician to provide the care necessary for my child's well being. I assume responsibility for all costs connected with the treatment.

3) I grant permission for my child to travel to and from ministry events with an adult leader. (Christmas Caroling etc.) Any such activity will be clearly communicated with me beforehand. Transportation will be provided via church van or leader vehicles. All drivers are required to pass a driving background check and posses current insurance.

4) I give permission for my student to be photographed or video recorded and for said photographs or videos to appear on church bulletin boards, slide shows, promotional materials, web & social media, for Belgrade Alliance Church.

5) I understand there are fees involved with participation in Awana. For 2025/2026 those fees include:

All Clubbers - Handbook $15
Cubbies & Sparks - Vest $15
T & T - Shirt $20
Cubbies & Sparks - Optional Book Bag $10

With all fees, books & uniform costs, this averages to approximately $3/week. If these fees are a financial hardship for your family, please let us know. We don't want finances to be a barrier to your child participating. Partial scholarships and payment arrangements are available. Fees will be collected at the Awana Block Party or the first night of club. You may also pay your fees online at http://belgradealliance.com/awana
